Last night, I had the pleasure of another visit to the Jazz Corner on Hilton Head Island. Great food and drink with wonderfully amazing music. Last night was more Motown with Reggie Deaz than jazz, but wow, what a night. Not being musical by any means, I am fascinated that these musicians can play for hours without referencing a sheet of music. The sounds from the guitars, the drums, the piano and the voice--truly unbelievable. To have such a gift and to share it night after night is special. Playing to packed house in a club ranked among the top 100 jazz clubs in the world while making everyone feel great; it doesn't get any better.

Yesterday, I read an article in Compliance Today magazine entitled, "The role of compliance in the quest to improve (the) patient experience." The gist of the article is that hospitals need to be careful with the inducements that we provide in forgiveness of financial obligations beyond financial hardship or purchasing items or services on behalf of the patient. This is a new area of concern that our compliance officer is addressing as our doctors and nurses work to get patients well enough to be discharged from the acute care and into the most appropriate care setting. In some cases, we provide items and services for the patient to facilitate the transition into the alternate care setting. As the article states, "while striving to improve (the) patient experience, we must recognize the legal limitations....." Sound advice in this era of false claims, anti-kickback violations and over payment for services. The article is worth the read in this ever changing complex field of health care.

Being a board member of Allegany College of Maryland, I have the distinction of being the part of tomorrow's graduation ceremonies. It is a wonderful accomplishment for the graduates as well as their professors and instructors.
I have been holding onto an article that I received a few weeks ago from my daughter. The article was written by Charles Wheelan and was published in the Wall Street Journal on April 30th; it is entitled, "10 Things Your Commencement Speaker Won't Tell You." Some of the 10 that are worth sharing are as follows: 2) Some of your worst days are ahead of you - you will face periods of self-doubt and failure, but work through them. 3) As you attempt to change the world, don't make things worse - do the right thing; you don't have to cure cancer but don't spread it either. 6) Read obituaries - these are short biographies that demonstrate that successful people rarely lead linear lives. 7) Your parents don't want what's best for you - they want what's good for you. As parents we want what is safe and comfortable; sometimes you have to take risks. 9) It's all borrowed time - don't take anything for granted; live life to the fullest. 10) Don't try to be great - the less you think about being great the more likely it is to happen and being solid isn't bad.

Health Leaders magazine did a survey throughout the health care industry recently. When focusing on clinical quality improvement, one of the components of Health Care Reform's Triple Aim along with reducing the cost of care and focusing on population health, 30% of hospital and health system CEO's responded that care coordination along the continuum of care is their greatest challenge. Interestingly enough, 25% of all groups surveyed also responded that care coordination was their number one challenge with improving the patient experience as a close second with 21%. CEOs also had the patient experience as second at 17%.
At WMHS, we are pulling out all stops to better coordinate the care of the patient. We have reorganized our Care Management department to better address overall care coordination of our inpatients. We have strengthened patient education and discharge planning. We have a care team lead by our Chief Medical Officer rounding on patients with a length of stay greater than 5 days to ensure that all that can be done for the patient is being done while they are an inpatient. We are working to enhance the communication process between our Hospitalists and physicians and other caregivers. We also have nurses rounding on patients with physicians and shift report taking place at the patient's bedside. At WMHS, we continue to make progress in all of these areas and I was pleasantly surprised to see that care coordination has become such an industry priority.

A friend of mine had a great line after last week's vote by the Maryland General Assembly to raise taxes. He said, "On Tuesday, I went to bed middle class and when I woke up on Wednesday, I was wealthy." Not only did the House and the Senate raise taxes on everyone making over $100K as a single filer and for any joint filers making over $150K, but they also shifted 25% of the pension costs for teachers to the counties. County governments across the state will in turn be forced to raise taxes locally. So, not only will we soon pay close to 9% for state income taxes, but the counties will increase their taxes to cover the pension shift. Wait a minute, aren't we the same taxpayers? So why is the State telling us about the money that they saved us by reducing the state's pension costs? Come January 2013, if the previously approved tax cuts aren't continued, the triple whammy will hit with an increase in the Federal income tax rate. What all of this tells me is that we are in store for a much deeper recession as our disposable income becomes significantly reduced.

Yesterday, our investment advisors were at WMHS with their semi-annual report on the health system's investments. Each gave a perspective on the market outlook for the remainder of this year and the next several years. The most surprising comment was that we shouldn't see any improvement until 2015 to 2018. According to their outlook, the current and future risks are many: a great deal of political uncertainty both in the US and abroad; a projected $500 billion in new taxes looming this coming January 1 which will impact disposable income; an economic slowdown in China; ongoing debt issues where the US government is addressing the debt issue by creating more debt (huh?). There is some good news with a few positives: US corporations are getting stronger; inflation continues to be low; aside from oil, energy costs are low; and the housing supply is improving but the housing market is still trying to bottom out. Using a health care analogy to describe our economy, one investment advisor said that it's like a patient who is no longer critical but remains in a weakened state, vulnerable to relapse. I wish the news was better, but at least we have the necessary information to plan accordingly.

They’ve been indicted. She’s been destroyed.
▪ The Department of Natural Resources caught a record-breaking 18-pound Snakehead in a tributary of the Potomac River in Blue Montgomery County, Maryland, where the median home price is $833,400. Snakeheads apparently have no fear of the property tax rate.
I checked the Potomac home town portal for news of the super snare but the last update was about the Great Falls Park flooding on May 22, 2011. I reckon the webmaster’s gone fishin’.
▪ More coddling for juvies this week when a Juvenile Court judge “sentenced” the youths involved in the early March killing of 13-year-old Monae Turnage. They covered her body plastic trash bags and left her there to die.
The 13-year-old shooter could serve up to three years in detention where he will be getting treatment and lots of store brand Jell-O. He’ll get out in time to get a DMV Learner’s Permit.
The 12-year-old accessory after the fact has been sent to live with relatives out of town under “juvenile services supervision.” No word on how long he’ll be on their dime. Juvenile records are sealed for our protection. It’s better not to know because there’s nothing we can do about it.
While Monae is spending the rest of her life in heaven and her family and friends mourning the tragic loss, the police investigation continues. A relative of one of the “children” sentenced yesterday has to ‘splane how the gun used in the crime was found under his bed. Then they have to find out why the weapon was found in a fellow officer’s car, and ask one of the baby’s mommas how her DNA got on Monae’s body.
▪ Obama discussed gay marriage with Michele and the children. Obviously, they were very persuasive. How quickly the children have embraced the Sidwell Friends School PC curriculum.
▪ Ironically, the Obama’s campaign manager is calling for Romney to show “moral leadership”.
▪ BO is taking some time off from the campaign trail to host weekend world summits.
He and some of the other world wizards in the G8 will gather at Camp David where Europe’s debt crisis will be the number one item on the “hoowah, bushwah, and blah blah” agenda.
▪ No “Smores around the campfire for Putin. Vlad’s skipping the summit and waiting to see who wins the U.S. election before he plays his next hand.

It's time to comment on the upcoming special legislative session scheduled next week to deal with a single issue: to raise taxes on Marylanders. There are reasons on both sides of the aisle not to allow the doomsday budget to take effect this fiscal year. However, my blog is about the absurdity of our elected leaders, all from the same party (Democrats), not finalizing the budget, which includes raising taxes, during the regular session. Instead, many of these senators and delegates, who will be running for re-election in 2 years, will have to vote to raise taxes in a spotlighted special session. This is a great time for any prospective challenger to begin preparing his or her campaign. Maryland, according to Governor O'Malley, has some of the lowest taxes in the country. Really? By raising the state income taxes to almost 9% that will place Maryland in the top 10 of states with the highest state and local income taxes. I guess the good news is that we don't live in France. Their newly elected President wants to raise taxes up to 75%. Get ready for the brain drain from France.

In November 2011, the Frederick County Board of County Comissioners made it a bit easier to build in Frederick County. But what does this mean exactly?
According to Sharon Goetz, Manager of Permitting Sercice for Frederick County Department of Permits and Inspections, homebuilders can now save about $500-$1000 per single family dwelling permit, and commercial developers can now save thousands of dollars related to building permits for commercial new construction due to the reduction of the Excise Tax to a $0 amount.
Background: The Excise Tax, prior to being reduced to a $0 amount, was charged on (1) all new dwelling units based on the square footage of the dwelling and (2) all new nonresidential floor area (new buildings, new additions, accessory structures, but not permits for existing floor area, like tenant fit outs). The rate for Nonresidential Excise Tax was $0.75 per square foot of floor area (per floor), so you can see how that added up quickly. Sometimes the Excise Tax for a new Nonresidential permit was tens of thousands of dollars! There were only a few exemption categories:
A. Farm construction
B. Government construction
C. Residential addition construction
D. Residential accessory structures

Weekly Business Visits Off To a Soaring Start

The BOCC’s new initiative for conducting business appreciation visits weekly kicked off in February with two exciting business visits. These visits are to continue throughout the year and are organized by Sherman Coleman, Business Development Specialist for the Business Development and Retention Division of Frederick County.
The first visit was conducted at the Sheriffs’ Youth Ranch in Frederick County. President of the Board of County Commissioners Blaine Young along with Commissioner C. Paul Smith,, joined Business Development Advisory Vice-Chairman Drex Ryberg and Acting Director Helen Riddle and Business Development Specialist Sherman Coleman for a tour of the facility and adjoining property. They met with Executive Director Mark Grover who has held that position for the last five years. Mr. Grover explained how the ranch that was purchased back in 1974 and sits on 204 acres of land. The site is licensed to house 18 young men, but currently has 11. The youth ranch is geared toward helping troubled youth become productive members of society. The focus is on preparation for the residents to be able to become more educated and distance themselves from a checkered past.
There has been tremendous success and with an average GPA of current residents at 3.24 Mr. Grover takes great pride in what his facility has been able to contribute to the Frederick community and the help that the Frederick Community has provided to the facility.
The second visit was held at the Frederick Athletic Club and the participants were BOCC President Blaine Young, County Manager Dave Dunn, BDAC Vice-Chairman Drex Ryberg and Business Development Specialist Sherman Coleman.
The visit and tour was hosted by Herman King. Mr King was just recently named Camp Director of the facility. The facility was built in 1999 and was opened in 2000. It is owned and operated by Mr. Jeff Neuman. The facility is 24,000 square feet which includes a full size basketball court. The facility has nine employees which includes a personal trainer. A child care center is on the premises and is managed by four individuals. Membership is just above 2,500 and the facility will host its annual Stu Vetter basketball camp which runs April 22nd through July 29th.

The contingent was hosted by President Patricia Motter and VP of Business Development Travis Hardmon. This meeting was set up as part of the BOCC's weekly business visits conducted by Business Development Specialist Sherman Coleman.
IHA has been headquartered in Frederick for the past 21 years. They are responsible for 1100 housing units. Their focus is on rehab and smaller development. They directly address workforce housing for teachers, firefighters, policemen, first responders and seniors.
The Alliance has been at it's current location for 12 years and operate with a staff of 12 full-time employees and two part-time and most recently two interns from Hood College.

This visit is part of the new weekly business visits initiative that the BOCC started last month. Also present at the meeting was Acting Director Helen Riddle of the Frederick County Business and Retention Division and Heather Gramm Business Development Specialist for the City of Frederick.
The school was established in 1868 and the museum was opened in 2008. The school is comprised of two campuses. One is located in Columbia while and the other in Frederick, with total enrollment of 492 students. The Columbia campus opened it's doors in 1972. The school uses a public school curriculum and offers a high school diploma.
Approximately 2/3 of the students live outside of Frederick County. Student enrollment is typically 450-500 students and Superintendent Tucker feels that is a goood number so there are no immediate plans for expansion.
The school consists of 65 acres and has 275 employees. It is also the 16th largest employer in Frederick County.
Superintendent Tucker was awarded the Lifetime Educator Award in March of 2011 at the Frederick County Chamber of Commerce 8th Annual Live Here-Work here awards ceremony at the Weinberg center.

The meeting was to discuss the open lines of communication between the bank and the County and City.
Frederick County Bank has five branches in the Frederick area and just celebrated ten years of doing business in Frederick this past October. Mr. William R. Talley Jr., Executive Vice President attributed the bank's success to "making and building relationships and treating people the right way". Mr. Clark Briggs, Assistant Vice President also added he feels that Frederick County Bank is an example of a "true corporate citizen" by consistently giving back to the community. One of the ways the bank give back to the community is through the volunteer efforts of its employees. There are 72 full time employees working for the bank and together they compiled over 400 community volunteer hours.

Tom Kleinhanzl, President and CEO of Frederick Memorial Hospital and Ken Coffey, Vice President and (Cheif Development Officer (CDO) were hosts for the weekly business visit held at FMH.
Mr. Kleinhanzl shared their recent improvements, which included the completion of the new "Wing C", and future plans for the hospital.
The hospital's core mission is acute care and there has been a 9% increase in patient enrollment over the past three years. There are approximately 1,700 re admissions a year. Mr. Kleinhanzl touched on how 40 millions dollars were spent on things such as new beds, the new parking garage, major physical renovations within the hospital and other routine expenditures.
The hospital is rated a level six out of seven on the IT scale involving electronic medical records, which ranks the hospital in the upper tier of new technology for hospitals.
According to Mr. Kleinhanzl community involvement is the key and giving back to the community is one of the ways the hospital accomplishes this goal. The hospital provides free blood work and radiology to the Mission of Mercy. Benefits given to the local community are in excess of 21 million dollars.
The hospital currently operates independently and does not see a need to merge with any other hospital in the future. Both Mr. Kleinhanzl and Mr. Coffey feel the future outlook for the hospital is bright.

Despite cool temperatures representatives from Frederick County Government and members from the Business Development Advisory Council were hosted by Catoctin Breeze Vineyard for a busines visit. Sherman Coleman and Colby Ferguson, both from the Business Development and Retention Division of Frederick County Government were also guests of Mr. Voytek Fizyta. The 24 acre vineyard has been in business for two years and was a dream of Mr. Fizyta when he arived in this country from Poland 31 years ago. Fizyta's grapes will be ready for harvesting later this year, but the Polish immigrant and electrical engineer has already made a variety of honey wines, traditionally known as Mead. The vineyard represents the ninth and newest Frederick County winery, and offers two table wines and three dessert wines.When Fizyta and his wife Alicja, looked to relocate from Potomac five years ago they immediately fell in love with the hilly terrain in Thurmont which provided the perfect spot for growing grapes because of the soil. Perched along hilly terrain, grapes get ample sunlight at Catoctin Breeze Winery. The constant breeze evaporates moisture which is a major problem for East Coast grape growers.The Fizytas outfitted their 1,200-square-foot basement to accommodate making wines. They expect their first significant harvest from the 2011 planting later this year. they are also planning to build a production facility on the premises in the near future. The owner Keith Marks stated the 500 square foot facility is a full service cycle shop offering services from sales to repairs. He currently has 35 bikes in all different styles and models.
